To apply for Ayushman Card, first you have to install ‘Ayushman Card App Ayushman Bharat (PM-JAY)’ in your mobile. After this the beneficiary will have to register by entering his mobile number. After this, the registration process will have to be completed with the help of OTP, Iris and Fingerprint and face-based verification. Meanwhile, some documents like ration card, Aadhar card, residence certificate, passport size photo, PAN card will also have to be uploaded. After this your information will be verified. First of all go to the official website of PMJAY https://pmjay.gov.in/. See the option of ‘Am I Eligible’ on the homepage. You will see this in the top menu only. There is also a question mark (?) mark before it, click on it.
As soon as you click, the login page will open in front of you. In this you will have to enter your mobile number. Generate OTP by entering the captcha code written nearby.
Look at the OTP that will come on your mobile and enter it in the prescribed field. After verifying the mobile OTP, you will have to select the state. Select the state in which you live.
After choosing the state, you will have to choose the category. You select the category through which you want to check your name. Some states provide the facility to check only by ration card number, while some states provide the facility to check the list by name or family number. Whereas in some states, there are options to search by mobile number, ration card and your name. You choose any one of the options given in your state.
After searching, you will know whether you are eligible to avail the benefits of this scheme or not. If your name is not included in the list of Ayushman Bharat Yojana, then No Result Found will be written in the search result box.
